Falcon Field Vehicle Access Card Application
The Falcon Field Access Cards are ready for distribution. Use this form to request Airport Vehicle Access Cards. Here are some things you need to know prior to applying for your card(s):
  • Each tenant whose name appears on the FFZ storage rental agreement will be allowed one (1) access card at no cost. Additional cards may be issued for a fee of $40 per card. The tenant who receives additional cards will be held accountable for the actions of the individuals to whom the cards are given.
  • The tenant’s rental agreement account will be billed for any additional cards you request.
  • The tenant’s account must not be delinquent. Any delinquent payments must be received prior to issuance of your access card(s).
  • Prior to receiving an access card, each tenant must be signed on the Airport’s “new” rental agreement. The tenant must sign the new agreement, pay any applicable fees, and provide any required aircraft/registration information before the tenant will be issued an access card.
  • Ground lease tenants who have access to the Air Operations Area through their leased parcels will not be issued access cards.

By submission of this on-line airport vehicle access card application, I hereby affirm that I have read, understand, and agree to comply with the Falcon Field Vehicle Access Rules and Regulations. I further agree that my vehicle access privileges to the air operations area may be suspended or revoked for a violation of these Rules and Regulations, or a violation of any provision of an aircraft storage or storage room agreement, ground lease agreement, or any City of Mesa ordinance, rule or regulation, or any directive of the Falcon Field Airport.
I understand that it is my responsibility to provide current contact information to the City and to immediately notify the Falcon Field Airport Administration Office at (480)644-2450 if my Access Card is lost or stolen.
Furthermore, I authorize the City of Mesa to bill my account for the amount of ADDITIONAL access cards requested above.
