Our recent edition of the City of Mesa Employee Benefits Newsletter highlighted the expected mailing schedule for new medical and prescription drug ID cards for use in 2013. The CVS Caremark ID card production facility (located on the East Coast) just notified Employee Benefits that the facility is behind schedule on cards and related materials being printed and shipped to COM members and other employer groups. Unfortunately, we don’t expect the “permanent” ID cards to begin mailing until approximately January 4th. In the meantime, there will be a special mailing tomorrow, Friday, December 28th of a “temporary” letter that has new prescription drug ID information individualized for each covered employee and retiree. For those employees/retirees who did not make changes to medical plan coverage for 2013 – your 2012 Medical Plan ID card is still good to use for medical plan information until you receive your “permanent” 2013 card(s).
Remember also, that although you may not have your 2013 cards in hand by January 1st, all your eligibility information will be in the respective systems and available for access on that date. More importantly, you can go online to
www.caremark.com on January 1st and print your own temporary ID card if you need one. You can also contact Caremark Customer Care at 1-855-264-5048 for assistance with identification information and getting scripts filled (open 24 x 7 including holidays). The COM Employee Benefits Office will be open for benefits and eligibility verifications/information from 7am to 6pm on December 31st for both walk-in and phone service (480-644-2299).

RX ID: RX00XXXXX (the last 5 digits is your COM employee number if you are an in-state member; if you are an out-of-state member, your ID is a BCBS generated number that carries over from 2012, so you will find that information on your current OOS medical plan card)
NAME: Covered Employee or Retiree Name
