Additional CVS Caremark Cards

CVS Caremark has arranged to have additional prescription/medical plan identification cards sent out to all in-state members, and prescription only cards sent to all out-of-state members. The cards were sent to help with initial print and production delays, requests for additional cards for other covered family members, misplaced or discarded materials, or to provide cards to members who did not receive their initial cards.  You may have already received these “duplicate” or “new” permanent ID cards or will begin to see them in the mail shortly, as this entire production run completed shipping last Saturday.

All in-state cards should have the new prescription drug ID number, which is your standard 5-digit medical plan ID number preceded by the letters “RX” and two zeros (e.g.  RX0012345).  Out-of-state members have a 9-digit ID number that is generated by Blue Cross Blue Shield (e.g. 850000000).  PLEASE EXAMINE YOUR NEW CARDS CAREFULLY WHEN YOU RECEIVE THEM TO ENSURE THAT THE ID NUMBER FORMAT IS CORRECT FOR YOU BASED ON YOUR STATUS AS IN-STATE OR OUT-OF-STATE.  CVS Caremark has advised that there were some cards sent out from the print/production facility in a separate mailing (but in a similar time period) with the last number of the ID inadvertently missing. So, it is possible that some members will receive two separate sets of identification cards. These incorrect cards with less than 9-digits in the ID number should be cut up and discarded.

In addition, the ExtraCare Health cards that were featured in the January Employee Benefits Newsletter are still on the way. Please be on the lookout for these white key-tag cards and information brochure from CVS Caremark. These ExtraCare Health cards offer additional discounts to CVS Caremark members as an added perk.
