2013 Flu Vaccination Program and Benefits

City of Mesa Health Plan
Flu season is underway. It’s time to consider where/when you and your covered family members can get a flu shot under your City of Mesa Health Plan benefits. Members on the City of Mesa Health Plan can get a flu shot at 100% coverage, in several ways:
- Go to your BCBSAZ network primary care physician office - present your COM medical ID card
- Attend an in-network retail medical clinic who provides flu shots, located inside some CVS, Walgreens or other free-standing or store locations (Minute, Take Care, Mollen) – not the pharmacy part of the store, the clinic part of the store – present your COM medical ID card
- Starting October 1st, go to a participating pharmacy network location like CVS, Walgreens, Costco Pharmacy or many supermarket based pharmacies/independent pharmacies that are in-network for vaccinations – present your CVS/Caremark Prescription Drug ID card
- On October 23rd, enjoy the City of Mesa Employee Health and Benefits Fair and get your flu shot while there – present your COM ID badge or your COM medical ID card
